What to include
- Delivery date, time of arrival, delivery company name, driver name, and the purchase order number this delivery fulfills.
- Line-by-line check of items received: product description, quantity ordered versus quantity delivered, and condition on arrival (undamaged, damaged, or missing).
- Notes on any discrepancies: short shipments, wrong items, visible damage with photos, and whether damaged items were refused or accepted with reservation.
- Signature of the person who physically received and inspected the delivery, confirming the count and condition documented above.
- Storage location where the materials were placed on site, and any follow-up actions needed (return damaged item, reorder shortage, notify supplier).
